We thank you for taking a moment to come to our web site and complete an observation form for the officials that worked your game.  Our effort to go paperless will hopefully make it easier for you to complete the form, easier for us to receive information quickly, and save you some postage expenses.  It will also make the form available to you at all times and eliminate the need for officials to have to provide a card at the beginning of each game.  If at any time you are not sure of the officials' names, your Athletic Director will have that information available to him/her through the Arbiter site.  Should you have any questions, please do not hesitate to contact us.  When you have completed your form, please submit it to board11coachform@aol.com.  This address is also written at the bottom of the form.  Once again, thank you for your input.  We greatly appreciate your cooperation.
